services REQUEST/ SPECIAL EVENT REQUISITION

ATTENTION: THIS FORM MUST BE COMPLETED FOR ANY SECOND CALVARY RELATED EVENT BEING HELD AT SECOND CALVARY BAPTIST CHURCH, THE FELLOWSHIP HALL, THE ANNEX OR OFF-SITE.

Please complete the form below to request space for your upcoming event. All request must be made twelve to sixteen (12-16) weeks prior to your event. After you have submitted your event, our Office Manager will be in touch with you to confirm all details. No events should be promoted without this form being completed, accepted and confirmed.

Failure to do so, will result in your event not being scheduled or promoted. Please contact your ministry leader for more details.
